NURSE PRACTITIONER

Cutting Edge Consulting Group is looking for a Nurse Practitioner to work for a large Managed Long Term Care Company (MLTC) servicing patients in the Manhattan.

Since this position involves in - home visits, the ability to work autonomously and travel will be required. You should also be comfortable working with geriatric and/or hospice patients who have a variety of health matters.

Responsibilities:

  • Conduct in - home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening

  • Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick and foot exam (as appropriate)

  • Identify diagnoses to be used in the furtherance of treatment

Qualifications:

  • Active, unencumbered license to practice nursing in the assigned state

  • Nurse Practitioner Certification from either the ANCC, AANP or AACN as Adult, Geriatric, or Family Care NP

  • 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse

Responsibilities:

The Nurse Liaison will coordinate and facilitate referrals to Hospice care based on Hospice admission criteria; recommends alternate plans of care as appropriate. Serves as on-site Hospice resource to assist facility staff that refers patients to Hospice Care. Works under general supervision.

Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required.

Education: Associate's Degree in Nursing required. Bachelor's Degree in Nursing or other health related field preferred.

Experience: Minimum of 2 years of clinical experience, preferably in a Hospice or Palliative Care Program, or Oncology services, required. Experience in sales, marketing management, administration, education and/or discharge planning preferred.

Responsibilities:

The instructor conducts training, in-services and competency evaluation programs for Home Care paraprofessionals in accordance with the New York State Department of Health. The instructor ensures that Home Care paraprofessionals are sufficiently knowledgeable and clinically competent to meet certification standards and tests trainees to evaluate effectiveness of training. The instructor reports on the progress of paraprofessionals under guidance during training periods.

Qualifications:

*Must be a graduate of an accredited school of nursing; Bachelor’s degree (B.A.) in Nursing is preferred and have current NYS license and registration to practice as a Registered Nurse in New York State.
